I appreciate all the time and effort Dr. Berwick has spent to both repair an ailing health care system and move beyond it into our future. I can only hope we heed his warnings and move beyond it into a healthier and more efficient future.
D Kortsch, MDD Kortschhttps://www.blogger.com/profile/14641592781052077059noreply@blogger.comtag:blogger.com,1999:blog-8852684919029181544.post-28497628887504349862011-12-15T05:26:10.807-08:002011-12-15T05:26:10.807-08:00Medicaid is unsustainable, esp now that Obamacare ...Medicaid is unsustainable, esp now that Obamacare increases it's ranks by 40% and takes millions of Americans off private insurance, into the high risk program. Yet Berwick says:

Among patients, put the poor and disadvantaged first – those in the beginning, the end, and the shadows of life. Let us meet the moral test.

If he really believed that, then he must have felt like a whale out of water. The task of "doing everything" to improve patient care for everybody is immense. It may now simply be a national "dream", but wasn't it nearly the same "dream" our forefathers had more than two centuries ago? Can we afford our nation's future without continuing the fight to make it a reality?Bill Wrightnoreply@blogger.comtag:blogger.com,1999:blog-8852684919029181544.post-85972905626126316672011-12-12T01:31:22.121-08:002011-12-12T01:31:22.121-08:00Dr Berwick is exactly right on every point. The one point he could make more directly is the distorting role of excessive money-making in the current system. It is all too easy, even without armies of expensive lobbyists, for commercial enterprises, non-profit organizations which have lost their mission, and individual medical professionals, executives, and entrepreneurs to become very rich. Don't believe for a minute that this is good for patients, as it is at the roots of the damaging practices that Dr Berwick describes.Anonymousnoreply@blogger.comtag:blogger.com,1999:blog-8852684919029181544.post-22729504836616994242011-12-11T15:57:54.402-08:002011-12-11T15:57:54.402-08:00It is so rare to hear anyone speak out in support ...It is so rare to hear anyone speak out in support of the patient. What a gift this was to read today. As a Northern Virginian living among so many involved in politics and pro-business factions, I admit I have become cynical because dialogue such as this rarely gets coverage. I would be dead if it was not for Medicare, which allowed me to seek out competent surgeons in other areas of the U.S. to get the diagnosis and surgery I needed to begin to regain my health -- the health I lost while insured by Cigna private insurance whose policies remunerate doctors for having "healthy" patients whether they really are or not. That subject appears to be taboo in the U.S.Anonymousnoreply@blogger.comtag:blogger.com,1999:blog-8852684919029181544.post-89942608202381929892011-12-11T05:30:22.106-08:002011-12-11T05:30:22.106-08:00I love the theme of 'all in' - total commi...I love the theme of 'all in' - total commitment to to total victory with the clarification that we can't 'cut' our way to fair and effective care without failing our moral obligation.Anonymousnoreply@blogger.comtag:blogger.com,1999:blog-8852684919029181544.post-17753597353645364042011-12-09T09:37:46.454-08:002011-12-09T09:37:46.454-08:00are we ready for Occupy Healthcare? I would add that we are all dancers.

My thought while reading this is how VITALLY important it will be to enlist those on the front lines - those actually doing the work - to enlighten those making policy. All too often, changes come from the top down without consulting those who actually know how systems work and the resultant changes are at best irrelevant, and not uncommonly, problematic.

Thanks to Don for all his work, and also gratitude to those whose guidance he received.
